Overview:
Are you a compassionate professional dedicated to making a difference in your community? Nurturing Care is seeking experienced Housing Navigators , Day Habilitation Specialists , and Care Managers to join our team. As a patient-centered organization, we are proud to be part of the CalAIM initiative, specifically the Enhanced Care Management (ECM) and Community Supports (CS) Program. This initiative focuses on providing high-quality care to Medi-Cal members insured with Molina, ensuring exceptional support for underserved populations.

In this hybrid role , you will combine telephonic outreach with essential face-to-face interactions to build supportive relationships with our members. Your efforts will specifically address the needs of populations such as:

  • Adults and families experiencing homelessness
  • Adults and/or children at risk of avoidable hospital or emergency department utilization
  • Adults and/or children with serious mental health and/or substance use disorder (SUD) needs
  • Adults and/or children with intellectual and developmental disabilities (I/DD)
  • Adults and/or minors who are pregnant and postpartum

What We Offer:

  • Day Habilitation Services: Provide members with support to build independent living skills, such as money management, communication, and accessing community resources, while promoting social engagement and personal growth.
  • Housing Navigation Services: Assist members in securing safe and stable housing by conducting housing searches, coordinating with landlords, submitting housing applications, and facilitating financial assistance such as deposits.
  • Case Management Services: Deliver holistic support to members by conducting assessments, creating individualized care plans, coordinating resources, and fostering collaboration among healthcare providers, social services, and the members’ support system.

Roles and Responsibilities:

  • Outreach and Engagement: Conduct in-person and telephonic outreach to Medi-Cal members, providing a warm, empathetic approach to establish and maintain positive relationships.
  • Risk and Needs Assessment: Complete thorough risk assessments and intake assessments to evaluate members' overall health, housing status, and independent living skills.
  • Care Planning: Develop and implement individualized care plans based on comprehensive assessments, ensuring alignment with members' physical, mental, and emotional needs and preferences.
  • Day Habilitation Support: Guide members in developing essential life skills, accessing community resources, and achieving greater independence.
  • Housing Navigation: Help members locate and secure housing, coordinate financial assistance, and address barriers to obtaining stable housing.
  • Ongoing Support: Offer continuous guidance to members, their interdisciplinary care team, and their support system, including motivational interviewing and behavioral health counseling.
  • Collaboration: Work closely with healthcare providers, community stakeholders, social services, and care teams to ensure seamless care transitions and comprehensive support.
  • Documentation: Maintain accurate member records and provide clear, detailed documentation of all interactions and care plans.
  • Caseload Management: Handle a large caseload (maximum of 50) with efficiency and adaptability, ensuring each member receives the attention and resources they need.

Qualifications:

  • Education: Associate or Bachelor’s degree in Healthcare, Social Services, or a related field, or a minimum of 3 years of complex case management and or housing navigation experience in lieu of a degree.
  • Experience: At least 1 year of experience in social work, behavioral health, housing services, or day habilitation support within San Diego County.
  • Transportation: Reliable source of transportation with active car insurance; must have no DUIs or other driving restrictions.
  • Skills: Strong commitment to high-quality care, excellent organizational and communication skills, and the ability to adapt and collaborate effectively with a diverse team.
